Feel free to print these at home and bring them with you when you come with your pet, or you can also fax or email them to us . Print both "Waiver form and Pet Information Sheet" . If you're wanting to use the pool only, then please only do the pool waiver form. If you're wanting your dog to attend daycare, stay with us overnights sometimes, and use the pool, then please print all 3

These will take 3-5 minutes to fill out, (3 sheets) You are more than welcomed to come in and fill them out at the front desk as well! Just plan on being with us for a few extra minutes! We do apologize for so much paperwork, but it's for everyone's best interest!
